I am sorry if this question has been asking a zillion times before....What is the main cause of the slide not locking back when the last round is fired :confused::o:D

If you put an empty mag in and rack the slide does the slide lock back? The metal insert on the mag follower is supposed to push up the slide stop as it comes up after last round is fired. Slight chance rounds are not strong enough to cycle slide all the way but probably not since all rounds cycle. The gun might still be tight and just needs more breakin. That's all my feeble mind can think of. I'm interested to find out what it is. Good luck.

The slide lock spring could be damaged and not holding the the slide lock pin in place.
I don't know if you read the manual or watched the Kahr video, but that explains how to take the pistol apart and put it back together with attention to inserting the slide lock and getting the spring in the correct position.
When the slide doesn't lock back on empty magazine, check to see that the slide lock is fully inserted. If the spring is damaged from inserting the pin, it may not stay in the pin's detent and hold the slide lock pin in the pistol, allowing the slide lock to back out a bit. This will cause the slide lock not to engage and may cause failures to feed.

My PM9 has the same problem. Occasionally the slide doesn't lock back after the last round. This only happens during firing. If I put an empty magazine into an empty gun, it is fine. The magazine seems to push the slide lock and the slide does lock back when cycled manually. Today it happen with a Cor Bon +P which was clocked at 1250 ft/s, so it is not for lack of recoil, and it cycles fine.
Also, once upon inserting a fresh magazine after the previous one emptied, the slide released it self without me touching it or the stop. I just kicked the but of the fresh magazine in, and the slide was released and chambered the first round. It was not a problem for shooting, but I think this may be a safety issue.

If the slidr lock is up in the notch it is under tension from the recoil spring pulling it forward. I can't see how inserting the mag in can push the slide stop down. If things line up just right I suppose slamming the mag in somehow hits the breechface just right to push the slide back just a bit to let the stop down so the slide comes forward.
