Long Story Short Here. I have a fracture in a lower vertebrae and some bulging discs. I've always carried IWB and AIWB. The pain is to the point I can't carry in my normal methods.
I carry a CM9.
What is a really good OWB or alternative method holster that someone recommends?

For the most part, these days, I carry a small gun in a pocket. It makes life easier, less irritating and painful, yet well protected. I too have bad discs and when herniated I have to drop anything on a belt, including the belt. I use a pocket holster for my Seecamp. That works. I also invested in a couple shoulder holsters. The key here is to stay with a small, lightweight gun as well.

A google search for "deep concealment holster" might give you some ideas: shoulder holsters, belly bands, smart carry, cell pal...

Most of the time with back issues as you describe it isn't the holster but the belt. A good holster with a poor belt doesn't work so it's a tough spot.
If you can handle a decent belt I'm a devout fan of Mernickle Holsters. With a CM9 the average person would forget it's there. I like the PS6, it rides very high so it conceals real well. I carry my 1911 in one and it's just the best for me.
For you it may not be. An IWB with a loose belt might help, you have to keep the pressure off the sore spots or you'll be in agony which will make you real cranky and irritable like Muggsy.

On my bad back days (cane, walker or back brace) I pocket carry in very baggie shorts or pants. AHolster is my preference.
My back brace makes a IWB or OWB impossible, I've tried everything including in the top of the brace, like woman carrying between her breasts. TERRIBLE!!
Only baggie pants pocket carry seems to work.

Front pocket...................Desantis Nemesis............................been carrying that way for 5+ years. If I can pocket an MK9, you can pocket a CM.................
Option 1: A belly band can provide deep concealment while also providing lumbar support.
Option 2: A lumbar back brace may be adapted with a pocket to hold your pistol.
Both can easily be concealed under a T-shirt, polo shirt, etc. and if untucked, will allow quick access to your pistol.

In addition to all the above suggestions,I have found the pouch holsters to be more comfortable for me than the Belly bands. I have the Pistol Wear PT-2 (made more for active wear under a sweat shirt) and the Tacti-Pac. I would also seriously consider the 3-Speed Holster and possibly the cheaper Smart carry. I have found the 12:00 position comfortable for me. I have seen the Ghost Holster but did not care for it and do not have one.

I will +1 on a good belt. It makes a huge difference. I use DalTech Force belts.

+1 on AHolster. I love mine, works great with sweats or gym shorts too. The design snags for a clean draw every time.

I carry a CM9 in a kydex front pocket holster and it's the most comfortable way for me to carry. The holster always stays in my pocket when I quick draw.........except the other day. I was wearing some cargo shorts with large pockets and holster/gun both came out. Tried it again and again. Same results. I was shocked. These were cargo type shorts, but the gun was NOT in the lower zip type pocket. I've never had problems with any others, even baggy fleece type warmups. So if you are going to carry front pocket make sure you check out every pair of pants/shorts.

This is why I use the Desantis Nemesis. After a while, it fit forms to your pocket, but never closes up. I have never had any trouble, wearing any kind of pants. My firearm always comes out easily, and the holster always stays put. I can see how it wouldn't work, in large cargo pockets. Any front pocket holster needs some material pressing against it, to work.
