Well it took almost 1000 rounds and a bit of tuning but my PM40 has finally settled down. Took it to the range yesterday after my smith worked some magic and put 125 rounds of several different types of ammo through it without a single malfunction. Previously, it had numerous problems with the slide locking open with ammo in the magazine. Found that the ammo was tapping the slide release. The smith relieved a bit of metal from the slide release and from the extractor and now she seemingly will run flawlessly. It loves Federal HST 165s and I think that will be a great defensive load.
I put Agrips on it and added Pearce grip extensions on my 5 round magazines. The Agrips are are great. They really make the gun much more comfortable to shoot. The Pearce extensions really make that snappy .40 more controllable. I love this gun! I also bought a Crossbreed IWB. It's awesome. You forget the pistol is on your hip!
Finally, my gun is performing as it should! Couldn't be happier!
